ABSTRACT
Background: The HIV/AIDS epidemic remains a public health concern in Africa particularly Sub-Saharan Africa. As at the end of 2017, among the 36.9 million people living with HIV world-wide, 1.8 million were under 15years of age. HIV infection prevention, particularly mother-to-child transmissions are key models of reducing the infection. The purpose of this study was to assess the implementation of the National Guidelines for PMTCT of HIV in selected districts in the Eastern Region of Ghana. Specifically the study sought to examine the context, processes, content, actor behavior and how these influence the implementation of the PMTCT guidelines. Method: The study employed a case study design with both qualitative and quantitative methods to assess the implementation of the national guidelines on PMTCT in health facilities in some selected districts in the Eastern Region. Fifteen (15) health providers were interviewed and 50 questionnaires administered to mothers with HIV. Content analysis technique was used to make sense of the qualitative while ANOVA, means and percentages were used to analyze the quantitative data. Results: The study found that although the guideline document was precise as well as userfriendly, compliance to the national guidelines on PMTCT was low. This was because of several contextual factors such as culture of the people, lack of resources and personal beliefs of the healthcare professionals; poor actors’ knowledge and understanding of the guidelines due to lack of training and retraining. Conclusion: In the two selected districts in the Eastern region, implementation was found to be low mainly due to the policy development process, contextual factors and actors’ poor iii knowledge of the policy. Effective communication and education on the guidelines to both clients and professionals is necessary for increasing adherence.
